Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Social Worker to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive social work services to patients across various settings, including hospitals, outpatient clinics, and community-based programs. Your expertise will support patients and their families through complex emotional, social, and practical challenges related to health conditions, end-of-life care, and recovery processes. The ideal candidate will demonstrate strong clinical skills, cultural competence, and a commitment to enhancing patient well-being through evidence-based practices.

• On Call/Call Back Requirements Can be low censused and put on call. Call back minimum is 2 hours
• Required Certifications to be Attached upon
Submittal:
WI License
•Master's Degree in Social Work (MSW) preferred
•
Minimum Experience:
1 yrs experience in social services and counseling in a hospital acute care setting with a diverse population

Med Surg and Critical Care Clusters
•all inpatient units
•Layered Model working in tandem with RN Team for Discharge planning Patient Ratio 8-20 per day based on acuity of patients
