Job Summary We are seeking a compassionate and dedicated Medical Social Worker to join our dynamic healthcare team. Develop individualized care plans that incorporate behavioral therapy, crisis intervention, and community resource coordination. Provide clinical counseling services such as child & family counseling, addiction counseling, and mental health support tailored to each patient's circumstances. Facilitate group therapy sessions and conduct psychotherapy using evidence-based techniques like cognitive behavioral therapy and motivational interviewing. Collaborate with interdisciplinary teams—including physicians, nurses, therapists, and case managers—to ensure seamless discharge planning and post-acute care. Maintain accurate medical documentation in electronic medical records (EMR) systems while adhering to HIPAA regulations and social work practice standards. Coordinate with community resources to assist patients with end-of-life care, hospice & palliative medicine needs, assisted living arrangements, and other supportive services. Skills Proven experience in medical social work within inpatient, outpatient, hospice, or nursing home settings. Strong knowledge of clinical research methodologies, diagnostic evaluation tools (ICD-10/ICD-9 coding), and medical terminology. Expertise in working with diverse populations including geriatric patients, children with developmental disabilities or autism, individuals with dementia or PTSD care needs. Skilled in crisis management, intake procedures, case management, and discharge planning processes. Proficiency in utilizing EMR systems for documentation and data collection; familiarity with CPT coding preferred. Excellent client communication skills combined with the ability to collaborate effectively within clinical interprofessional teams. Ability to handle sensitive situations with empathy while maintaining confidentiality according to HIPAA guidelines. Experience working with individuals from diverse cultural backgrounds in settings such as home & community care or outpatient clinics is highly valued.
