We offer comprehensive services including primary care, OB-GYN, vision, behavioral health, and patient navigation. We seeking a compassionate Licensed Clinical Social Worker (LCSW) to join our behavioral health team. We serve a diverse community with culturally sensitive mental health services aimed at improving overall well-being.

Position Summary:

The LCSW provides clinical behavioral health and medical social work services, including assessments, therapy, referrals, and care coordination, both in-person and via telehealth. Working closely with primary care providers, you will help co-manage patients with medical and psychiatric needs, triage urgent cases, and refer patients to specialty mental health services as appropriate.

Key Responsibilities:

Deliver individual, family, and group counseling for patients aged 12+ Develop and implement therapeutic treatment plans and document all clinical activity in the EMR Provide crisis intervention and outreach in community settings such as schools and shelters Collaborate with providers to support integrated care and patient-centered treatment planning Conduct community presentations on mental health topics and promote available resources Track patient follow-up and maintain compliance with clinic and accreditation standards

Qualifications:

Licensed Clinical Social Worker (LCSW) credential in California Master's degree in Social Work; community clinic experience preferred Bilingual in English and Spanish strongly preferred BLS Ability to work effectively with diverse populations in a fast-paced environment

Schedule :

This is a full-time position following an alternative work schedule (4/10)

Work Setting:

In-clinic and off-site location such as a shelters, school, FRC or other community settings
