Initial Requirements:

• Must have Masters Degree in Social Work
• Must have Home Health experience
• Minimum one year clinical experience (two+ years preferred)
• Current license to practice in CA
• Current CPR certification
• Current CA driver's license plus proof of car insurance.
• Computer/Internet experience (web-based charting to be used)
• Bi/Multi-lingual skills a plus (e.

g. Spanish, Farsi, Tagalog, Chinese etc.) Responsibilities Conduct comprehensive patient assessments to develop individualized care plans. Provide clinical counseling and behavioral therapy to patients, utilizing techniques such as cognitive behavioral therapy and motivational interviewing. Ensure compliance with HIPAA regulations while maintaining accurate medical documentation and records. Collaborate with healthcare teams to coordinate patient care and facilitate home health services. Engage in crisis intervention and support families through child and family counseling as needed. Maintain proficiency in medical terminology and computer skills for effective patient management.
