Job Description
Summary:
The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicare-approved transplant centers. As a member of the transplant team, the Solid Organ Transplant (SOT) Social Worker utilizes specialized clinical knowledge and advanced clinical skills to help in the areas of assessment, treatment of medical, patient and caregiver navigation, mental health and behavioral disorders, and discharge planning. This position supports all programs within the departments, including Adult Congenital Heart Disease, Advanced Heart Failure, Adult Heart Transplant, and VAD. Hospital Experience preferResponsibilities:
Ensures attendance at multidisciplinary rounds or huddles and reports on psychosocial status and its implication for transitions of care.Participates in the Transplant Selection Committee; presents the mental health, social status, and functional abilities of candidates.
Conducts a detailed psychosocial assessment to determine the candidacy for solid organ transplant or living donation.
Participates in discharge planning including coordinating patient transfer to other facilities.
Monitors progress toward the goals of the social work plan of care and revises it in response to changes in patient needs and condition.
Organizes and facilitates care conferences with patients, families, representatives, and members of the multidisciplinary care team.
