Job Overview We are seeking a compassionate and energetic Licensed Clinical Social Worker (LCSW) to join our dynamic healthcare team. In this vital role, you will deliver mental health support, counseling, and social services to patients across our community. Duties Conduct comprehensive mental health assessments and develop individualized treatment plans for patients Provide counseling sessions to address issues such as depression, anxiety, trauma, and behavioral health concerns Collaborate with healthcare providers to coordinate patient care and ensure holistic treatment approaches Maintain accurate medical documentation and record patient progress in electronic health records (EHR) systems Educate patients on mental health topics, coping strategies, and available community resources Report patient conditions accurately and promptly communicate any urgent concerns to the medical team Support patients in navigating social services, housing, employment, and other community resources Skills Valid NC licensure as a Licensed Clinical Social Worker (LCSW).
