Job Description

Essential Duties and Responsibilities Conduct comprehensive psychosocial assessments and evaluations. Develop individualized treatment plans based on patient needs and goals. Provide individual, family, and/or group therapy services. Diagnose and treat mental health and behavioral health conditions within scope of licensure. Provide crisis intervention and stabilization services as needed. Maintain accurate and timely clinical documentation in the electronic medical record (EMR). Coordinate care with primary care providers, specialists, hospital staff, schools, community agencies, and other healthcare professionals. Assist patients with coping strategies, behavioral modification, stress management, grief counseling, trauma support, and other therapeutic interventions. Identify social determinants of health impacting patient outcomes and connect patients with appropriate community resources. Participate in multidisciplinary care planning and quality improvement activities. Maintain patient confidentiality and comply with HIPAA and all applicable federal and state regulations. Maintain professional licensure and continuing education requirements. Qualifications Master's Degree in Social Work (MSW) from an accredited program required. Current Licensed Clinical Social Worker (LCSW) license in the State of Nevada required, or ability to obtain prior to employment. Experience in outpatient behavioral health, integrated care, rural healthcare, or community mental health preferred. Strong interpersonal, communication, and clinical assessment skills. Ability to work independently while collaborating within a multidisciplinary healthcare team. Experience with electronic medical records preferred.
