We are seeking a compassionate and skilled Medical Social Worker (MSW) who recognizes that hospice care is more than a service—it is a sacred opportunity to support individuals and families as they navigate serious illness, loss, meaning, and transition. Position Summary The Medical Social Worker provides emotional, psychosocial, and practical support to hospice patients and their loved ones. As an integral member of the interdisciplinary team, the Social Worker helps patients and families identify strengths, address concerns, access resources, and find stability and connection during a profoundly important season of life. Responsibilities Conduct comprehensive psychosocial assessments for hospice patients and families. Develop individualized plans of care in collaboration with the interdisciplinary team. Provide supportive counseling related to illness adjustment, anticipatory grief, caregiver stress, and end-of-life concerns. Assist patients and families in identifying and accessing community resources, financial assistance programs, and long-term care options. Facilitate advance care planning discussions and support patient autonomy and goals of care. Provide education regarding hospice philosophy, available services, and community resources. Participate actively in interdisciplinary team meetings and contribute to care planning. Support bereavement and grief services as appropriate. Maintain timely, accurate, and compliant clinical documentation. Participate in quality improvement activities and uphold hospice regulatory standards. Qualifications Master's Degree in Social Work (MSW) from an accredited school of social work. Current licensure in the State of Oregon preferred (LMSW, CSWA, or LCSW). Minimum of one year of healthcare experience preferred; hospice, palliative care, home health, or geriatric experience strongly desired. Knowledge of end-of-life care, grief processes, family systems, and community resources. Excellent communication, counseling, and organizational skills. Ability to work independently while collaborating effectively within an interdisciplinary team. Valid driver's license, reliable transportation, and proof of automobile insurance.
