Job Description

Olivera Health Group is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our integrated healthcare team. This role is ideal for a behavioral health professional who is passionate about improving patient outcomes through collaborative, evidence-based care in a primary care setting. The LCSW will provide in-person behavioral health services with the flexibility of a hybrid work schedule, partnering closely with primary care providers to deliver comprehensive, patient-centered care. Responsibilities Conduct mental health screenings, psychosocial evaluations, and comprehensive assessments for medically referred and self-referred patients. Provide short-term, structured, evidence-based psychotherapy as part of individualized treatment plans. Collaborate closely with primary care providers and other members of the healthcare team to improve patient health outcomes. Deliver individual and group therapy for patients experiencing mental health and co-occurring disorders. Coordinate referrals to specialty behavioral health services, substance use treatment programs, and community resources when appropriate. Provide crisis assessment and intervention as needed. Advocate for patients by helping address barriers to care through follow-up, care coordination, and collaboration with community partners. Maintain accurate, timely clinical documentation in accordance with organizational and regulatory standards. Participate in interdisciplinary care planning and quality improvement initiatives. Qualifications Active Illinois Licensed Clinical Social Worker (LCSW) license required. Master's degree in Social Work (MSW) from an accredited program. Strong interpersonal, verbal, and written communication skills. Ability to build rapport with patients from diverse backgrounds. Experience in integrated healthcare, medical social work, primary care, or community mental health preferred. Experience with evidence-based behavioral health interventions and crisis management is preferred. Pay:
$80,000.00 - $95,000.00 per year
