Our planned services include intensive outpatient programming, group and individual therapy, psychiatric medication management, case management, skills training and coordinated recovery support. Our model emphasizes trauma-informed, culturally responsive and person-centered care. Elore Health's Oregon Health Authority Certificate of Approval application is currently pending. This is an anticipated position. Any offer and clinical start date will be contingent upon OHA certification, satisfactory license and credential verification, background clearance and applicable payer-enrollment requirements. Position Summary Elore Health is seeking a compassionate and experienced Oregon Licensed Clinical Social Worker to provide individual and group therapy within our adult behavioral health and co-occurring treatment program. The LCSW will facilitate structured IOP groups, complete clinical assessments, develop person-centered service plans and collaborate with psychiatric, clinical and case-management staff. The ideal candidate is comfortable treating adults with mental health and substance use disorders in a hybrid care environment. Key Responsibilities Conduct comprehensive biopsychosocial and mental health assessments. Formulate

diagnoses within professional scope of practice. Develop, review and update individualized, measurable service plans. Facilitate structured mental health and co-occurring IOP groups. Provide individual therapy and other clinical interventions as indicated. Utilize evidence-based approaches, including
CBT, DBT
skills, motivational interviewing, relapse-prevention strategies and trauma-informed care. Complete suicide, self-harm, violence and other clinical risk assessments. Develop safety plans and coordinate higher levels of care when clinically necessary. Maintain timely, accurate and compliant assessments, progress notes, service plans and discharge documentation. Coordinate care with PMHNPs, the Clinical Director, SUD treatment staff, QMHAs, case managers, peer-support staff and outside providers. Participate in treatment-team meetings, clinical consultation, quality-improvement activities and staff training. Provide services through both in-person and secure telehealth modalities. Follow

Part 2, Oregon Administrative Rules and Elore Health policies and procedures. Required Qualifications Master's degree in social work from a CSWE-accredited program. Current, active and unrestricted Oregon Licensed Clinical Social Worker license. At least two years of relevant post-graduate behavioral health experience preferred. Experience conducting assessments, developing treatment plans and providing individual or group psychotherapy. Knowledge of suicide-risk assessment, crisis response and safety planning. Strong clinical documentation, organization and communication skills. Ability to work effectively with adults from diverse cultural, racial, socioeconomic and lived-experience backgrounds. Ability to complete required background and exclusion screenings. Ability to participate in onsite services at Elore Health's Portland location. Preferred Qualifications Experience providing adult intensive outpatient or partial hospitalization services. Experience treating co-occurring mental health and substance use disorders. Training in CBT, DBT, motivational interviewing, trauma-informed care or relapse prevention. Familiarity with ASAM Criteria and recovery-oriented treatment. Experience providing group therapy.
