The LCSW will provide clinical consultation and guidance to Lead Care Managers and other care team members serving Medi-Cal members with complex medical, behavioral health, and social needs. This position focuses primarily on clinical consultation, assessment, care planning, and care coordination rather than ongoing psychotherapy. Primary Responsibilities Provide clinical consultation and case support to Lead Care Managers and members of the interdisciplinary care team. Review complex member cases involving behavioral health conditions, substance use, homelessness, family concerns, safety risks, or other significant psychosocial barriers. Assist with psychosocial assessments, behavioral health screenings, risk assessments, and individualized care planning. Help identify appropriate behavioral health, substance use, medical, social service, housing, and community-based resources. Support referrals and coordination with mental health providers, substance use treatment programs, hospitals, primary care providers, specialists, managed care plans, and community agencies. Participate in interdisciplinary case conferences and provide recommendations regarding member goals, risks, barriers, and appropriate interventions. Support members during transitions from hospitals, emergency departments, skilled nursing facilities, behavioral health facilities, and other care settings. Provide guidance regarding crisis situations, safety planning, mandated reporting, and escalation to emergency or behavioral health services when appropriate. Educate and support staff regarding trauma-informed care, motivational interviewing, behavioral health conditions, and effective member engagement. Review and provide clinical input on member assessments and care management plans. Document clinical consultations, recommendations, and member-related activities accurately and within required timeframes. Maintain compliance with HIPAA, confidentiality requirements, professional standards, managed care plan requirements, and Whole Care Solutions policies. Perform other ECM-related clinical duties as assigned and within the LCSW's professional scope of practice. Minimum Qualifications Active and unrestricted California Licensed Clinical Social Worker license. Master's degree in social work from an accredited program. Experience working with individuals who have complex medical, behavioral health, substance use, or social needs. Strong clinical assessment, documentation, communication, and care coordination skills. Ability to work collaboratively with nurses, social workers, care managers, healthcare providers, managed care plans, and community organizations. Knowledge of Medi-Cal, CalAIM, or Enhanced Care Management is preferred. Experience with crisis intervention, homelessness, serious mental illness, substance use disorders, or high-risk populations is preferred. Schedule Per Diem hours based on program and member needs. Some remote work may be available depending on assigned responsibilities.

$62.00 - $72.00 per hour
