Medical Social Worker
Job Description
Overview The Medical Social Worker is a key member of Oak Street Health Centers, supporting the psychosocial needs of patients through assessments, advocacy, and care coordination. Responsibilities Provide comprehensive assessments, telephonic check-ins, and in-person visits. Partner with patients and support persons to identify goals and drive care plan execution. Maintain knowledge of community resources and make referrals as needed. Navigate patient insurance benefits with the Patient Relations Manager. Provide comprehensive care coordination to empower patients. Educate patients on supportive care options and provide goals of care counseling. Provide psychoeducation on life stage and disease-specific issues. Collaborate with care teams to generate solutions to patient issues. Complete concise and comprehensive documentation on time. Partner with Community Health Workers for clinical guidance. Respond to crises and urgent needs, prioritizing work effectively. Skills & Qualifications Fluency in Spanish, Polish, Russian, or other languages as needed. Proficient computer skills including Microsoft, GSuite, and electronic medical records. Experience Prior experience with complex care coordination or population healthcare management. Minimum one year of medical social work experience in a healthcare environment.
