Job Description

Position Summary The Clinical Social Worker provides counseling, psychosocial assessment, and discharge planning for respite participants, supporting transitions to stable housing, recovery programming, and follow-up care. Key Responsibilities
  • Provide evidence-based individual counseling to participants
  • Conduct psychosocial assessments on admission and throughout the stay
  • Respond to behavioral health crises and provide intervention
  • Develop and coordinate discharge and transition plans
  • Connect participants to housing, recovery, and community-based services Qualifications
  • LICSW or LCSW licensure in Vermont (or license-eligible)
  • Experience with SUD, trauma-informed care, or homeless/underserved populations preferred
  • Strong care coordination and case management skills Physical Demands
  • Primarily an office/counseling setting with some walking within the facility
  • May involve emotionally demanding crisis situations Work Environment A 6-bed residential medical respite facility in Burlington, VT, working as part of an interdisciplinary clinical team.
