Job Description

SUMMARY:
Seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) for a 13-week travel assignment. The LCSW will provide comprehensive psychosocial assessments, discharge planning, care coordination, and counseling services for adult inpatient medical patients. The ideal candidate will collaborate with interdisciplinary teams to ensure patients receive appropriate resources and support for successful transitions of care. Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments for hospitalized patients.
Develop and implement individualized discharge plans in collaboration with the healthcare team. Coordinate post-discharge services, including home health care, medical equipment, rehabilitation, and long-term care placements. Participate in interdisciplinary care conferences and treatment planning. Provide individual, marital, and parental counseling as appropriate. Assist patients and families in navigating community resources and public assistance programs, including Medicaid, Medicare, TANF, Food Stamps, AFDC, and medication assistance programs. Coordinate referrals to community agencies and healthcare providers. Facilitate communication among patients, families, physicians, nursing staff, and community partners. Assist with Department of Children and Families (DCF) processes when applicable. Manage cases involving adult and child protective services, guardianship, power of attorney, emergency detention, and other legal or protective concerns. Support patients with behavioral health and substance use concerns by coordinating appropriate treatment resources. Document all assessments, interventions, care plans, and discharge coordination activities accurately in the electronic medical record (Epic). Float between assigned units as needed to support patient care. Required Qualifications
  • Active Washington State Licensed Clinical Social Worker (LCSW) license.
Current Basic Life Support (BLS) certification. Minimum of one year of LCSW experience. Experience with electronic medical records (Epic preferred). Strong knowledge of discharge planning, care coordination, and community resource management. Excellent communication, critical thinking, and interdisciplinary collaboration skills. Any Specific Mandate skill required
  • Adult inpatient medical social work.
Psychiatric hospital experience. Experience with involuntary holds (5150), behavioral health, or substance use treatment. Knowledge of adoption, foster care, and protective services. HL01
Pay:
$2,100.00 - $2,200.00 per week Application Question(s):
