Job Summary We are seeking a dedicated and experienced Substance Use Disorder Program Manager to lead Capstone Health's Substance Use Disorder (SUD) and Medication-Assisted Treatment (MAT) program. The Program Manager delivers evidence-based behavioral health services to individuals with substance use and co-occurring mental health disorders while coordinating integrated care with medical providers, behavioral health clinicians, Peer Support Specialists, and community partners. This position is responsible for program coordination, clinical assessment, treatment planning, staff supervision, quality improvement, regulatory compliance, and performance monitoring to ensure safe, effective, patient-centered care. The Program Manager promotes recovery-oriented, trauma-informed, and culturally responsive services while supporting Capstone Health's mission and commitment to improving health outcomes in underserved communities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Clinical Services Develops, implements, evaluates, and updates individualized treatment plans for the provision of mental health/SUD and support services jointly with the Psychiatrist, Medical Providers, and other staff members as appropriate. Performs comprehensive bio-psychosocial assessment for mental health, substance abuse, co-occurring disorders, and medical needs as needed, and conducts follow up as appropriate based on results of evaluation. Schedules and completes ASAM assessments on MAT patients as needed. Conduct crisis intervention, suicide risk assessments, safety planning, and appropriate referrals. Collaborate with medical providers to support integrated treatment of co-occurring medical and behavioral health conditions. Coordinate referrals to inpatient, residential, outpatient, and community-based behavioral health services as appropriate. Provide patient education regarding Medication-Assisted Treatment (MAT), recovery resources, program expectations, treatment requirements, and relapse prevention strategies. 8. Assess psychosocial barriers to treatment participation and coordinate referrals to appropriate community resources and support services. Medication-Assisted Treatment (MAT) Program Management Coordinate patient intake and referral processes for the MAT program. Conduct initial patient outreach, provide education regarding MAT program expectations, assess readiness for treatment, identify barriers to participation, and coordinate scheduling of required assessments and appointments. Collaborate with prescribing providers to support safe and effective medication-assisted treatment. Monitor patient engagement, adherence to treatment plans, and continuity of care. Coordinate patient follow-up activities and support retention in treatment. Ensure ASAM assessments, required screenings, and clinical documentation are completed in accordance with organizational policies and regulatory requirements. Collaborate with nursing staff, providers, Peer Support Specialists, and care teams to ensure coordinated patient care. Monitor program performance measures and identify opportunities for program improvement. Conduct routine MAT patient check-ins to assess medication adherence, side effects, withdrawal symptoms, cravings, behavioral health status, psychosocial needs, and treatment engagement, communicating significant findings to the interdisciplinary care team. Assess patient readiness for recovery using evidence-based motivational interviewing techniques and the Stages of Change model, documenting findings and coordinating appropriate interventions. Observes urine drug screen collections in accordance with MAT Program protocols, organizational policy, and chain-of-custody requirements. Leadership and Program Administration Provide day-to-day operational oversight of the SUD and MAT programs. Supervise, mentor, coach, and evaluate assigned Peer Support Specialists and other designated staff. Conduct regular supervision meetings and document supervision activities. Assist with staff orientation, training, and professional development. Provide ongoing consultation, coaching, training, and operational support to Peer Support Specialists while monitoring performance, documentation quality, and adherence to program requirements. Promote a collaborative, recovery-oriented, and patient-centered culture. Participate in strategic planning and program development initiatives. Develop and maintain collaborative relationships with community organizations, referral partners, and behavioral health providers. Communicate performance concerns, compliance issues, patient safety concerns, and professional development needs to the Behavioral Health Director in a timely manner. Documentation and Reporting Document patient outreach, MAT intake activities, behavioral health screenings, supervision activities, patient education, urine drug screen activities, referrals, care coordination, and follow-up in accordance with organizational documentation standards. Ensure documentation complies with organizational policies, payer requirements, licensing standards, and applicable federal and state regulations. Prepare required program reports, grant reports, quality metrics, and performance data. Monitor program outcomes and assist with data collection for organizational and grant reporting. Part 2, and applicable confidentiality requirements. Quality, Risk Management, and Patient Safety Participate in organizational quality improvement, performance improvement, compliance, patient safety, and risk management initiatives. Promote evidence-based practices and continuous quality improvement within the SUD program. Monitor clinical quality indicators and patient outcomes. Identify and communicate patient safety concerns, compliance issues, operational risks, and barriers to care through established reporting processes. Report incidents, adverse events, compliance concerns, and potential risk exposures in accordance with organizational policies and procedures. Participate in audits, accreditation reviews, corrective action initiatives, and performance improvement activities. Support compliance with HRSA requirements, FTCA risk management expectations, 42 CFR Part 2, HIPAA, state licensing requirements, and organizational policies. Community Outreach and Collaboration Develop and maintain relationships with community agencies, behavioral health providers, hospitals, law enforcement, recovery organizations, and social service agencies. Promote awareness of behavioral health and substance use disorder services. Participate in community education, prevention initiatives, and outreach activities. Support efforts to improve access to behavioral health services for underserved populations. Collaboration and Professionalism Demonstrate professionalism, integrity, and ethical clinical practice. Promote the mission, vision, and values of Capstone Health. Participate in interdisciplinary team meetings and care coordination activities. Complete required continuing education and maintain professional licensure. Perform other duties as assigned.

MINIMUM QUALIFICATIONS

EDUCATION Required Master's degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a related behavioral health discipline. Required Licensure Current unrestricted Alabama licensure as one of the following: Licensed Independent Clinical Social Worker (LICSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT) Licensed Psychologist

EXPERIENCE

Required Minimum of three (3) years of post-licensure behavioral health experience. Minimum of one (1) year of experience providing substance use disorder treatment. Experience conducting behavioral health assessments and treatment planning. Experience working with individuals experiencing co-occurring mental health and substance use disorders. Preferred Supervisory or program management experience. Medication-Assisted Treatment (MAT) experience. ASAM assessment experience. Federally Qualified Health Center (FQHC) or integrated care experience. Experience with grant-funded behavioral health programs. Knowledge, Skills, and Abilities Knowledge of substance use disorders, co-occurring disorders, and evidence-based treatment practices. Knowledge of ASAM Criteria and DSM-5 diagnostic standards. WORK ENVIRONMENT

Work is performed primarily in an ambulatory healthcare and integrated behavioral health environment. Frequent interaction with patients experiencing behavioral health and substance use disorders. Frequent collaboration with medical providers, behavioral health clinicians, Peer Support Specialists, and community partners. Occasional travel between clinic locations and community agencies may be required. Occasional evening or community outreach activities may be required.

Ability to remain stationary for extended periods while providing counseling and completing documentation. Ability to move throughout clinic and community settings.
