CORNERSTONE CARES
, our new in-home behavioral health program serving children, adolescents, and families throughout the Naugatuck Valley . CARES is designed for children and adolescents who need more support than traditional outpatient therapy or a structured transition from a higher level of care. Clinicians provide planned, family-centered treatment with increased clinical contact, caregiver involvement, and care coordination. This is not a crisis or on-call role. This position is ideal for an LMSW, LPCA, or MFTA looking for meaningful child and family work, flexible scheduling, regular (paid)clinical supervision, an enhanced hourly rate for in-home services, and mileage reimbursement. Conduct behavioral health intakes and ongoing clinical assessments. Develop and update individualized treatment plans based on clinical needs and measurable goals. Support emotional regulation, coping, communication, parenting strategies, family routines, and behavioral change. Coordinate care with authorized providers and community partners, including DCF, IICAPS, hospitals, IOP programs, PCP offices, schools, and outpatient providers. Complete clinical risk assessments and safety planning when clinically indicated. Maintain timely and accurate electronic clinical documentation. Travel throughout the Naugatuck Valley for scheduled in-home services. Participate consistently in clinical supervision, case consultation, program training, and quality review. Qualifications Master's degree in Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or a qualifying related discipline. Current Connecticut LMSW, LPCA, or MFTA license. Experience working with children, adolescents, families, community behavioral health, or higher-acuity outpatient populations preferred. Comfortable providing services in family homes and community settings. Strong clinical documentation, communication, organization, and time-management skills. Valid driver's license, reliable transportation, and current automobile insurance. Bilingual English/Spanish/Portuguese skills preferred but not required.
