  • Responsibilities:
  • + Is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.
+ Is knowledgeable about disease, treatment, social and economic implications for individuals and families as it pertains to their age, stage of psychosocial development and discharge needs. + Is pragmatic in approaching End of Life and ethical issues. + Utilizes clinical counseling skills when applicable.
  • Minimum Qualifications:
  • Required Education:
  • Masters in Social Work or Mental Health Counseling.
  • Required Experience:
  • 1 year social services in a community based social services or health care setting.

  • LMHC (Mental Health)
