Develop and implement individualised treatment plans. Provide individual, family, and group therapy services. Facilitate addiction recovery and relapse prevention programmes. Deliver crisis intervention services when needed. Maintain accurate and timely clinical documentation. Collaborate with physicians, case managers, and treatment teams. Participate in interdisciplinary treatment planning meetings. Support discharge planning and continuity of care initiatives. Required Qualifications Master's Degree in Social Work, Counselling, Marriage & Family Therapy, or a related field. Active Kentucky License as one of the following:

LCSW LPCC LMFT

Minimum three years of experience in one or more of the following: Mental Health Behavioural Health Addiction Recovery Substance Use Treatment Strong assessment, counselling, and treatment planning skills. Excellent communication and documentation abilities.
