The Behavioral Health Hospital Liaison serves as the primary connection between behavioral health providers, hospitals, and other healthcare facilities to ensure effective coordination of care for individuals with mental health and/or substance use disorders. This role facilitates timely referrals and supports transitions between inpatient and outpatient services to promote continuity of care and reduce hospital readmissions. Primary Duties and Responsibilities Collaborate with hospital staff to identify patients who may benefit from behavioral health services Coordinate the referral process for patients transitioning from the hospital to behavioral health facilities Provide education and support to hospital staff on behavioral health resources and services Ensure timely and accurate communication between hospitals and behavioral health facilities Monitor and track patient outcomes to ensure continuity of care Assist with data entry, reporting, and documentation required for compliance or quality assurance initiatives. Advocate for clients to access necessary services, including housing, substance use treatment, medical care and benefits. Works effectively with all patients and families to promote a milieu of trust while providing excellent mental health treatment. Participates in required trainings. Attend all scheduled team meetings. Completes all other assignments or duties as designated. Qualifications High School Diploma/GED required. Bachelor's or Master's degree in social work, psychology, counseling or a related field is preferred. 2 years of Previous experience in Behavioral Health, case management, or Hospital Liaison work preferred. Valid Michigan Driver's License and Vehicle Insurance Experience with Behavioral health and substance use programs preferred. Skills Required Strong communication skills Data entry skills with a high degree of accuracy, efficiency and timeliness Ability to work collaboratively with hospital staff and behavioral health professionals The ability to provide culturally competent services that shows sensitivity to the service population's cultural and socioeconomic characteristics. Working Conditions Clinical office environment; Non-Traditional hours may vary Travel between hospital sites and agency locations is required Working with consumers with a variety of behaviors, including aggression May involve working in high-stress clinical settings and handling sensitive or emergent situations This description is intended to describe the type and level of work being performed by a person assigned to this job.
