Jamestown Community Learning Council's (JCLC) mission is to strengthen the capacity of families to nurture, support, and provide for their child's academic development, emotional growth and physical well-being by establishing effective relationships between home and school. JCLC has been providing home visiting services that support families across Chautauqua County for over 35 years using the Parents as Teachers (PAT) home visiting model and is proud to offer a Healthy Families New York model program - Healthy Families Chautauqua. Using the Parents as Teachers (PAT) curriculum, a Family Support Specialist supports families by focusing on parent-child interactions, development centered parenting and family well-being. In a culturally sensitive way, partners, facilitates, and reflects with families to help them reach their goals and understand they are their child's first and best teacher. This 37.50 hr/week year-round position We offer paid training in the Healthy Families NY and PAT curriculum. We offer 12 days PTO and paid holidays. Core Responsibilities :

Maintain a caseload of 15-20 families based on

HFA/HFNY

requirements Engage and recruit families prenatally and with children up to 24 months old Participate/attend outreach events and efforts Help parents and children transition to other services needed Provide group connections to give families an opportunity to build social connections Connect families to resources that help them reach their goals and address their needs Plan, deliver, and document home visits based on the family needs Complete developmental screenings (ASQ3 and ASQSE) with parents and make referrals as needed Utilize the parent education curriculum within home visits Participate in professional development and trainings for model implementation and to increase knowledge and skill development Complete work and program requirements in a timely fashion based on HFNY deadlines

Qualifications:

Associates degree in early childhood education, social work, health, psychology or related is preferred, will consider 2 years supervised experience working with children and/or parents Knowledge of child development and milestones for children birth- 5 years old Computer skills: Word, Excel, Google Docs, web-based database, and email Ability to work independently and be self-motivated Ability to establish rapport with families and empower them by building on strengths Strong organization record keeping skills Drivers license and transportation JCLC is committed to fostering and maintaining a work environment where diversity, equity and inclusion are fully integrated into everything we do for the benefit of our employees and the families we serve.
