We have a large population of patients who suffer from mental health issues that receive primary care services at Hope Health Family. They have limited access to care. Recovery and wellness are uniquely defined for each person struggling with addiction. The needs of the people and communities we serve are the central aspect of our program. Objective perspectives take place as we slowly piece each client's puzzle together in order to use all available data to support their recovery process. Your responsibilities may vary based on the people you work with and the programs through which you provide services. You will coordinate closely with the interdisciplinary teams, including the mental health provider, to best help serve clients. Additional responsibilities include : Conducting clinical assessments Developing person-centered plans for service users Coordinating care and support within the community Assisting the client in all areas of need Some billing knowledge is preferred +++ This site is a Substance Use Disorder Treatment and Recovery Loan Repayment Program (STAR LRP) approved site.+++
