Clinical Practice:
Provide individual and group behavioral health counseling, including grief therapy, using virtual and on-site delivery models.
Assessment & Evaluation:
Conduct initial patient assessments, establish therapeutic goals, and continuously evaluate treatment progress and intervention effectiveness.
Care Coordination:
Partner with patients, families, physicians, and community providers to deliver collaborative care and resource referrals from admission through discharge.
Documentation:
Maintain thorough, confidential records of patient observations, treatment plans, and therapeutic outcomes.
Requirements & Schedule:
Requires a Master's degree (Counseling, Psychology, or Social Work) and LCSW for a full-time, 40-hour hybrid role (Monday-Friday, with occasional evenings for groups). What minimum requirements you'll need Licensure / Certification /
Registration:
One or more of the following required: Clinical Counselor credentialed from the Illinois Department of Financial and ProfessionalRegulation obtained prior to hire date or job transfer date. Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or transfer date. Licensed Clinical Professional Counselor (LCPC) credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date. Social Worker specializing in Clinical Practice credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date. BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
