We are seeking a Licensed Case Manager / Behavioral Health Clinician to join our team in providing high-impact, community-based care coordination and skill-building services in San Rafael, Tiburon, Sausalito, Mill Valley and surrounding areas . Position Overview In this role, you will work directly with individuals and families in their homes and community settings to foster independence and improve quality of life. This is a part-time, non-exempt W-2 position offering exceptional flexibility and highly competitive compensation.

Key Responsibilities Clinical Assessment & Intervention:

Monitor client functioning, conduct reality-testing, and perform solution-focused interventions for clients with severe and persistent mental illness.

Safety & Crisis Management:

Identify and intervene in high-risk situations, including thoughts of self-harm or inability to meet basic needs.

Skill Building:

Model and teach essential life skills including social interactions, money management, healthy living, and conflict resolution.

Resource Advocacy:

Assist clients with housing placements, benefit acquisition (SSI, SSDI, Medi-Cal), and linkage to vital community resources.

Collaboration:

Liaise with family members, medical providers, and stakeholders to ensure seamless care coordination.

Documentation:

Maintain precise records within the Electronic Health Record (EHR) using measurement-based care to track progress. Qualifications & Skills We are looking for an experienced professional who exudes cultural humility and has a passion for serving marginalized populations.

Education:

Master's degree or higher in Psychology, Social Work, or Counseling.

Licensure:

Active and unrestricted California license (LCSW, LMFT, LPCC, or PsyD/PhD).

Experience:

At least 5 years of dedicated experience working with serious mental illness (SMI) and/or co-occurring disorders.

Mobility:

Valid California driver's license, acceptable driving record, and access to a personal vehicle for community travel (with required insurance coverage).

Technical Proficiency :

Comfortable navigating EHR systems and mobile technology.

Soft Skills :

Impeccable communication, advocacy, and problem-solving abilities.
