Full licensure to practice independently in Arizona, including one of the following: Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW). No provisional or limited licenses; history of maintaining a clean professional record without disciplinary actions.
