We deliver coordinated case management, mental health rehabilitation, counseling, skills training, and family-centered supports to individuals and families dealing with mental health challenges, life transitions, and system involvement. Our focus is on stability, day-to-day functioning, and long-term independence. We are hiring a Qualified Mental Health Professional (QMHP) to join our multidisciplinary care team and work with clients on goal-directed, community-based behavioral health services. Position overview The QMHP provides rehabilitative and supportive behavioral health services under licensed clinical supervision. The core of the job is skills training, case management support, helping clients stabilize, and coordinating services, in close contact with our clinicians, client families, and community partners. This role suits someone who wants hands-on client work and likes being part of a team. Key responsibilities Deliver mental health rehabilitation and skills training services Support clients with independent living skills, emotional regulation, and social functioning Assist with treatment planning and documentation Provide crisis intervention and stabilization support as needed Coordinate services with medical, educational, housing, and community providers Support medication education and adherence in coordination with licensed staff Keep clinical documentation accurate, timely, and in line with state and Medicaid standards Take part in supervision, team meetings, and ongoing training Services you'll support Targeted Case Management Mental Health Rehabilitation Skills Training and Development Assessments and Service Planning Counseling Support (under licensed supervision)
