TITLE:
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)
DEPARTMENT

REPORTS TO
Behavioral Health/Social Services Manager
POSITION STATUS/HOURS
Part-time .8 FTE (32 hours/week) Monday - Friday hours in a clinic setting. Shifts would include 8a-4:30p three days a week and 10:30a-7p one day a week. Benefit eligible position!
JOB SUMMARY
This role will include providing mental health individual sessions to patients of varying ages in the Rural Health Clinic.
DUTIES INCLUDE

): Act as a catalyst for mental wellness, empowering individuals as a provider of mental health counseling Utilize your passion to cultivate health with your career and identify clinical criteria (DSM-5/ICD-10) for diagnosis and treatment of children, youth, adolescent/young adult, adult populations and couples seeking counseling Apply evidenced-based, innovative therapeutic modalities to treat patient's presenting disorders and increase symptom alleviation Develop treatment plans that align with client strengths and challenges, with emphasis on treatment modalities that are a best fit to client s DSM-5 diagnosis Complete timely documentation in accordance with state and ethical guidelines Work in collaboration with providers to promote overall health of patients who are referred for mental health services
QUALIFICATIONS
1. Master s Degree and license in Social Work (LCSW) or Ph.D. or Master s in Counseling or Marriage and Family Therapy 2. Current Wisconsin licensure as a Licensed Clinical Social Worker or Current Wisconsin licensure as a Licensed Professional Counselor
