Full-time| 40 hours | 8:00am-5:00pm How you'll make an impact in this role:
Patient Assessment & Care Planning:
Conduct initial evaluations to understand patient needs and co-create clear, actionable therapeutic treatment goals.
Therapeutic Intervention:
Deliver direct individual and group counseling sessions, continuously evaluating patient progress and adjusting therapy approaches to ensure maximum effectiveness.
Collaborative & Integrated Care:
Work side-by-side with patients, family members, physicians, and local providers to build a seamless, supportive care team from day one of treatment through discharge.
Community Connection:
Empower patients by connecting them with essential community resources, support networks, and therapeutic activities to extend care beyond the clinical setting.
Clinical Documentation:
Maintain meticulous, timely, and strictly confidential patient records and progress notes in compliance with professional and privacy standards. What minimum qualifications you'll need: Licensure / Certification /
Registration:
One or more of the following required: Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. BLS Provider preferred. American Heart Association or American Red Cross accepted. Supervisor credentialed from the Wisconsin Department of Safety and Professional Services preferred.
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required. Life at
