Responsibilities:
Provide case management services to clients in behavioral health setting Develop individualized service plans and coordinate care Connect clients with community resources, support services, and aftercare programs Collaborate with therapists, medical staff, and other team members to ensure comprehensive client care Maintain accurate documentation and comply with regulatory guidelines
Qualifications:
Master's degree (Required), (Preferred) in Social Work, Counseling, Psychology, or a related Behavioral Health field Licensure preferred (LMSW, LCSW, LPC, LMFT, etc.) or working toward licensure Experience in mental health, substance use treatment, or case management preferred Strong communication, organizational, and problem-solving skills Ability to work collaboratively in a team-oriented environment Why Join Us?
