Job Description
Behavioral Health Provider -
School- Based General Statement of Duties:
This is a position at Northwest Colorado Health as a Behavioral Health Provider (BHP),. A BHP provides integrated behavioral health as a key member of the primary care team. School-based BHPs (SB BHP) work within local school systems to provide direct patient care. The BHP abides by guidelines and expectations specific to the schools in which they provide patient care. Services will be provided within designated school sites overseen by the Community Health Center with a focus on evidence-based treatment of patients within the school system.Supervision Received:
Works under the direct supervision of the school-based Health Director with clinical oversite by the Behavioral Health Director.Supervision Exercised:
None Essential Functions:
1. Provides brief evidence-based treatment for psychiatric disorders and emotional/behavioral dysregulation that does not meet criteria for a psychiatric condition. 2. Provides evidence-based screening, assessment, treatment planning, and risk assessment of behavioral health conditions. As needed, the BHP provides case management, consultation, case coordination to facilitate referrals to specialty care, and crisis intervention. Remains current with standards of mental and behavioral health practice. 3. Coordinates care with Community Health Center and School systems. When coordinating with individuals outside of theCHC, BHP
is sure to align with all privacy laws and legal/ethical guidelines. 4. Participates in the Performance Improvement Process 1. Participation in program development as is needed by behavioral health team 2. Participate in the review on outcomes of individual client interventions as well as general program effectiveness. 3. Participates in performance improvement and quality improvement efforts. 5. Demonstrates dedication to meeting encounter targets on an individual and team level. Completes documentation and requirements supporting financial health of organization. PsyD/PhD in Clinical Psychology, Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Social Worker preferred. Proof of working toward licensure required. Licensure candidates may be considered.Experience:
Two years of experience working in a behavioral health setting preferred. Experience working in school systems preferred. Passion and ability for working with youth and their support systems necessary. Training in risk assessment required. Ability to ensure patients have a safe and supportive environment to seek treatment required. Bilingual preferred, not required.Requirements:
1. Active Colorado license or candidacy that allows you to work toward licensure under supervision by Licensed provider as defined by DORA in the state of Colorado. 2. Must be able to provide proof of adequate insurance coverage totaling at least $300,000 and valid driver's license. 3. Current CPR certification according to agency policy.Knowledge and Skills:
1. Knowledge and experience providing working within evidence-based assessment and treatment of psychiatric conditions and social/emotional/behavioral distress that does not meet criteria of a psychiatric disorder. 2. Knowledge of the problems and dynamics in treating medically underserved populations. 3. Awareness of multicultural considerations for care of patients being served. Driven to grow in knowledge to grown in cultural awareness in order to provide culturally informed care.Abilities:
1. Ability to communicate effectively, verbally and in writing. 2. Ability to establish and maintain working relationships with professional and paraprofessional medical personnel; patients; relatives and friends of patients; and the general public. 3. Ability to work successfully as part of interdisciplinary teams, including those within schools and the integrated primary care team.Equipment:
1. Daily use of telephone, computer, copier and other office equipment.Working Conditions:

The salary range for this position located in Routt County is $70,179.20 to $78,436.00. The rate listed is just one component of our total compensation package for employees.
