Job Description

Job Summary We are seeking a dedicated and compassionate florida Board Certified Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to ensure clients receive the necessary support. The Case Manager will work closely with clients facing various challenges, including behavioral health issues, child welfare concerns, and addiction recovery. Responsibilities Conduct thorough assessments of clients' needs and circumstances to develop effective case management plans. Provide crisis intervention and management support to clients in urgent situations. Collaborate with multidisciplinary teams to coordinate services and resources for clients. Mentor clients by providing guidance, support, and encouragement throughout their journey. Facilitate access to behavioral health services, workforce development programs, and child welfare resources. Utilize motivational interviewing techniques to empower clients in making positive changes. Maintain accurate documentation of client interactions, progress notes, and case files. Supervise support staff and interns as needed, ensuring adherence to best practices in case management. Requirements Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred. Proven experience in case management or related fields such as social work or behavioral health. Knowledge of crisis intervention strategies and techniques. Familiarity with child welfare systems and workforce development initiatives is a plus. Strong mentoring skills with the ability to motivate and inspire clients. Excellent communication skills, both verbal and written. Ability to work collaboratively within a team environment while also being self-motivated.
