Prioritize tasks effectively in high-pressure environments like Level I Trauma Centers or Level II trauma centers with acute care and emergency medicine focus. Communicate clearly with patients and their families to explain diagnoses, treatment options, and care plans while working with diverse cultural backgrounds. Participate in utilization management activities to ensure appropriate level of care and resource utilization for mental health services. Support patients through intake processes, diagnostic evaluations, and ongoing case management efforts within inpatient or outpatient settings. Assist with discharge planning by coordinating follow-up services and community-based supports for sustained recovery. Experience Proven experience working within hospital environments such as Level I Trauma Centers or Level II trauma centers with exposure to acute care settings. Hands-on experience in hospital medicine or emergency medicine units with familiarity in ICU or PICU settings preferred. Demonstrated background in behavioral health case management involving mental health counseling and working with individuals with substance use disorders or disabilities. Knowledge of medical terminology, physiology, and clinical case management principles essential for effective patient assessment and treatment planning. Experience utilizing electronic health records (EHR/EHR) systems while maintaining strict adherence to HIPAA standards for patient privacy. Prior involvement in community resource coordination related to social work or post-acute care services is highly valued. Strong task prioritization skills combined with excellent client communication abilities—especially when working with children or families—are critical for success in this role. Certifications such as Basic Life Support (BLS), CPR, and familiarity with addiction counseling or behavioral health interventions are advantageous but not mandatory. Responsibilities:

Position Summary The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.
