Targeted Case Managers Are you passionate about helping individuals rebuild their lives, achieve their goals, and access essential services? About the Role Targeted Case Manager (TCM) Targeted Case Management focuses on connecting individuals with mental health needs to community resources, coordinating services, and ensuring they receive the support necessary to achieve stability and independence.

Work Environment :

Community-based. Requires travel, home visits, and transporting members in agency vehicles.

Requirements :

Bachelor's Degree in a human services field Valid Florida driver's license with a clean driving record Reliable transportation and willingness to travel in the community Ability to obtain Certified Behavioral Health Case Manager (CBHCM) credential (Fellowship House covers the cost and provides support!)

Key Responsibilities :
