About Us Evolutions Treatment Center is seeking a Behavioral Health Case Manager to join our Miami treatment team. The Case Manager will provide comprehensive case management services to clients receiving substance use and behavioral health treatment. This position involves direct client support, court and probation coordination, assessments, documentation, benefits assistance, and connection to community resources. The ideal candidate is organized, compassionate, dependable, and comfortable communicating with courts, probation officers, treatment providers, and community agencies. Responsibilities Manage an assigned caseload of clients receiving detoxification and residential treatment services . Complete initial case management assessments and identify client needs, barriers, strengths, and available resources. Provide ongoing case management services throughout the client's treatment episode. Communicate with probation officers, courts, attorneys, and other outside agencies regarding client needs, treatment participation, and discharge planning. Assist clients with meeting court, probation, treatment, and other legal requirements. Facilitate communication between clients, the treatment team, probation, courts, and community providers. Assist with court-related coordination and provide appropriate documentation and updates when requested. Complete timely, accurate, and clinically appropriate case management notes and documentation in the electronic medical record. Assist clients with applying for SNAP/food assistance, Medicaid, Social Security, housing, transportation, identification, employment assistance, and other community resources. Connect clients with appropriate community resources and follow up regarding services. Coordinate referrals and continuing-care services prior to discharge. Assist with discharge planning for clients transitioning from detoxification and residential treatment to appropriate levels of care and community services. Advocate for clients and assist them in navigating behavioral health, substance use, social service, and criminal justice systems. Participate in treatment team meetings and communicate relevant case management updates. Maintain documentation and case management practices consistent with DCF requirements, The Joint Commission standards, and organizational policies. Maintain confidentiality and comply with all applicable federal, state, and organizational requirements. Participate in audits, reviews, and quality-improvement activities related to case management documentation and services as needed. Perform other duties as assigned. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, Criminal Justice, Sociology, Behavioral Health, or a related field preferred. Previous experience in behavioral health or substance use treatment , particularly detoxification and residential levels of care , preferred. Experience working in a treatment facility regulated by DCF and/or accredited by The Joint Commission is strongly preferred. Experience working with courts, probation officers, attorneys, or justice-involved individuals is a plus. Knowledge of behavioral health, substance use treatment, community resources, and discharge planning. Strong written and verbal communication skills. Excellent organization and time-management skills. Ability to manage multiple clients and deadlines while maintaining accurate and timely documentation. Comfortable communicating with clients, families, treatment providers, government agencies, courts, probation officers, and other community partners. Ability to maintain professional boundaries and confidentiality. Bilingual English/Spanish is a plus. Must be able to successfully complete all required background screening and facility onboarding requirements.
