Job Scope:

Provide community-based support to youth and families by connecting them to essential resources such as housing, food, transportation, and mental health services. This role supports the Mental Health Juvenile Justice (MHJJ) Program by coordinating care, developing goal-oriented treatment plans, and building collaborative relationships with the juvenile justice system, schools, and community partners. The Case Manager advocates for client needs, delivers life skills training, and ensures timely, accurate documentation of services provided. Primary Responsibilities Assists youth and family in the community via locating housing, food & transportation resources, applying for entitlements, providing counseling and life skill training and advocating for the client's needs Supports the Mental Health Juvenile Justice (MHJJ) Program and builds collaborative relationships between the Illinois Juvenile Court System, probation, schools, the community and Ecker Center ​to identify youth at risk for mental health concerns Provide training to collaborating agencies to understand the potential mental health needs of youth involved in their system Utilizes the MHJJ screening tool and works collaboratively with the individual youth, his/her family, the Managed Care Organization and the referring individuals to link the youth and family with any necessary community-based mental health and social services Develop goal oriented IATP's that are strength-based, and needs focused on all assigned clients as required Transport families as needed Provide accurate, quality documentation within a maximum of 48 hours after service delivery Provide support to the case management team and other programs as required Performance Requirements Demonstrate knowledge of documentation standards and capacity to meet documentation expectations. Ability to get clients to participate in individual sessions. Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session. Ability to clinically "listen" during session. Ability to establish rapport with clients and interact constructively with others in the workplace. Ability to focus client on individualized treatment plan and goals. Give information in a clear and understandable fashion and respond constructively to appropriate client questions and concerns. Education and Experience Bachelor's degree in human service field or equivalent experience One-year clinical experience Must have a valid Illinois driver's license, auto insurance, automobile, and a willingness to transport clients as necessary Schedule Tuesday 11:00am

• 5:30pm, & Saturday 9am
• 2:00pm, 35 hours/week.
