Overview We are seeking a Case Manager to coordinate the practical side of recovery for our residents: benefits, legal obligations, medical and dental care, housing, employment, and a warm handoff to the next level of care. You will carry a manageable caseload and be the link between our residents and the courts, agencies, and community providers they depend on. Key Responsibilities Complete case management assessments and individualized service plans within required timeframes Coordinate benefits, legal and probation obligations, medical and dental appointments, and other outside needs Build and maintain relationships with courts, probation/parole, DCBS, community mental health centers, recovery housing, and employers Develop discharge and continuing-care plans beginning at admission; secure step-down placements Facilitate life-skills and re-entry groups Provide family contact and coordination within consent guidelines Maintain accurate and timely electronic health record documentation Participate in weekly treatment team and utilization review meetings Uphold professional boundaries and trauma-informed practices Required Qualifications Bachelor's degree in social work, psychology, counseling, human services, or a related field; OR associate degree plus two years of behavioral health or human services experience Valid driver's license and insurable driving record Strong organizational and written communication skills EHR documentation proficiency preferred Preferred Qualifications Experience in residential SUD treatment, community mental health, DCBS, or court/probation settings Kentucky targeted case management or Medicaid documentation experience CADC, TCADC, or Kentucky Peer Support Specialist certification Lived experience of recovery

Pay:

$19.00 - $23.00 per hour Expected hours: 40.0 per week
