Job Description
SUMMARY:
The Case Manager will conduct patient interviews and assessments, including level of care appropriateness, risks, and strengths. The Case Manager will engage patients in their treatment, continuing care planning and discharge planning. This role requires knowledge of and experience with community resources and referral systems and/or the ability to work in collaboration with community-based providers and agencies. This role requires basic knowledge of addiction, and mental health recovery processes, services, and systems of support. The Case Manager will promote recovery through role-modeling and respectful interactions with patients and colleagues.Pay Structure:
Non Masters $24.00- $25.00 Masters non licensed or LCSW $26.00
