Cornerstone Behavioral Healthcare is seeking a dedicated Adult Case Manager to join our Behavioral Health Home Organization (BHHO) team serving Washington County, Maine. Position Overview As an Adult Case Manager/Health Home Coordinator, you will work closely with adults living with Serious and Persistent Mental Illness (SPMI) and co-occurring disorders. You'll help clients navigate services, build individualized goals, access resources, and strengthen both behavioral and physical health supports. This position offers a combination of community-based work, client advocacy, and care coordination. What You'll Do Build supportive working relationships with adults experiencing mental health and substance use challenges Complete and coordinate comprehensive bio-psychosocial assessments Develop individualized Plans of Care alongside clients and treatment teams Coordinate care with medical providers, behavioral health professionals, and community supports Assist clients with accessing housing, benefits, healthcare, transportation, and other essential services Support crisis planning, stabilization, and transitions between levels of care Advocate for client rights, independence, and recovery goals Participate in team meetings with clients, families, and providers Provide transportation assistance and support community-based activities when appropriate Maintain accurate and timely documentation in the Electronic Health Record system Complete authorizations and reporting requirements in compliance with MaineCare and DHHS guidelines Compensation & Benefits Competitive pay based on experience Bonus plan opportunities Mileage reimbursement at $.54/mile Flexible scheduling options Health, dental, and life insurance 401(k) with employer matching Paid time off Supportive supervision and ongoing professional development Qualifications Required MHRT/C or Provisional MHRT/C Certification Valid driver's license and current vehicle insurance (100,000/300,000 coverage) Ability to travel throughout Washington County and meet clients in home and community settings Strong communication, organization, and problem-solving skills Ability to maintain professional boundaries and confidentiality Comfort using electronic systems and basic office technology Preferred 1-2 years of experience working with individuals living with mental illness Familiarity with LOCUS assessments and SPMI populations Experience working within multidisciplinary treatment teams We are willing to train the right candidate who is motivated, compassionate, and eager to grow within the behavioral health field.
