Licensed Master's and Bachelor's of Social Workers wanted in Trenton, Michigan! PsyGenics Inc is a leading behavioral health care agency providing community service to vulnerable children and adults with intellectual and developmental disabilities or serious mental health issues in Wayne County. Position Summary The Case Manager is responsible for ensuring all service needs of a consumer are met by providing linking, coordinating, advocacy, negotiating and referral services. The Case Manager is responsible for the assurance of a Person Centered Plan as well as the monitoring of the established goals and the implementation of various treatment plans. Specific Responsibilities Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines Acts as the Chairperson of the multidisciplinary team Conducts self audits as well as peer audits Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures Coordinates and completes all necessary referrals on behalf of client Monitor all services provided on behalf of client Monitor satisfaction with services Provide resource development in an attempt to reduce the amount of service gaps Develop and maintain written records necessary for professional practice as defined in the procedural manual Provide outreach services to individuals and their families in their respective locales and/or homes as appropriate to their unit's function Provide community education services as assigned Participate/attend clinical supervision, team meetings and case consultations Provide services in the field as necessary Obtain all prior authorizations and applicable coordination of care Follow PsyGenics, Inc. professional code of conduct Represent PsyGenics, Inc. in the community Perform similar or related duties as assigned Must possess a master's or bachelor's in social work , be licensed in the State of Michigan in good standing as well as meet the criteria for a Children's Mental Health Specialist, Qualified Intellectual Disabilities Professional and Qualified Mental Health Professional. Person must have police and DHS clearance. Must have a minimum of one year experience working with children and adults with Developmental Disabilities, persons with a Traumatic Brain Injury or serious mental health issues.

a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain clinical relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative. The position is not remote. LMSW, LLMSW, LBSW or LLBSW required. Maximum rate of pay $67,000 per year . Salary commensurate with licensure and experience.
