HOW YOU'LL MAKE AN IMPACT
Mental Health Targeted Case Manager (TCM) provides home and community-based mental health services to adults with severe and persistent mental illness (SPMI), and children with severe emotional disturbance (SED) and their families. Establish relationships with assigned clients and supports and meets with each client as planned. Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy. Be familiar with implementation of health integration, including care coordination agency wide. Maintain current knowledge of the mental health service system with the ability to location information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs. At a minimum meet the monthly required contact for each client on caseload; document all services provided accurately, completely and within required deadlines.
WHAT YOU'LL NEED
Qualify as a Behavioral Health Practitioner (BHP) 4,000 hours working with those with mental health, developmental disabilities, or chemical dependency diagnosis: which can be part- time over several years or 2 years full time. Or Holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship or employment that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields (2,000 hours). Or Holds a master's degree or another graduate degree in behavioral sciences or related fields. Clear DHS background study Valid Driver's license and acceptable driving record. Complete initial and ongoing training requirements defined for the position to meet required standards. Clear DHS background study. Valid Driver's License and acceptable driving record.
WHAT WILL SET YOU APART
Experience working with individuals living with SPMI/SED or co-occurring disorders. Knowledge of community resources and experience connecting clients with needed support.
