Job Description

Position Summary Liaisons Community Care is seeking a Qualified Professional to join a Community Support Team serving adults in Wilson and surrounding communities. The QP provides person-centered planning, intensive case management, coordination, skill building, crisis support, and community-based interventions for adults with complex mental health, substance use, or co-occurring needs. Essential Responsibilities Coordinate initial and ongoing assessment activities and contribute to functional needs assessments. Develop, implement, monitor, and revise person-centered plans in collaboration with the consumer, team, providers, and natural supports. Provide psychoeducation, crisis support, relapse-prevention assistance, and behavioral or substance use interventions within scope. Provide intensive case management, linkage, referral, monitoring, and follow-up. Coordinate with medical, psychiatric, behavioral health, housing, vocational, legal, and community providers. Help consumers locate and maintain housing, connect to financial and in-kind resources, strengthen daily living skills, and prevent housing crises. Monitor progress toward goals and communicate intervention effectiveness to the Team Lead. Participate in weekly face-to-face team meetings and maintain working knowledge of all consumers served by the team. Participate in assigned crisis-response coverage and provide flexible services based on consumer acuity and need. Complete accurate, timely, Medicaid-compliant documentation. Required Qualifications Meets the North Carolina Qualified Professional definition under 10A
NCAC 27G
.0104 and applicable agency credentialing standards. At least one year of experience working with individuals with mental health or substance use disorders, as required for CST team members. Knowledge, skills, and abilities appropriate to the adult population served. Experience with person-centered planning, crisis intervention, case management, and community resource coordination. Preferred Qualifications Bachelor's or master's degree in a human services field. Prior CST, ACT, intensive case management, housing-support, or community-based behavioral health experience. Familiarity with NC Medicaid, Trillium requirements, and electronic health record documentation. Schedule and Availability This position is scheduled according to consumer needs and team coverage requirements. Candidates must be able to work a flexible schedule and participate in the team's assigned crisis-response and on-call rotation. Individual employees are not expected to remain continuously available; coverage is coordinated across the CST. General Employment Requirements Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area. Ability to complete required background, health, credentialing, and payer enrollment checks. Ability to maintain timely, accurate, and Medicaid-compliant documentation in the electronic health record. Ability to work collaboratively with adults who have complex mental health, substance use, housing, medical, safety, vocational, and legal needs. Completion of all CST model, crisis-response, evidence-based practice, and agency-required training within established timeframes. About the Service Community Support Team is an intensive, community-based service for adults with mental health, substance use, or co-occurring disorders and complex treatment needs. The team provides treatment, rehabilitation, intensive case management, service coordination, skill building, crisis intervention, and support for successful community living.
