Position Summary Liaisons Community Care is seeking a substance use professional to serve as the dedicated substance use member of a Community Support Team in Wilson. This position provides recovery-focused substance use interventions, relapse-prevention support, intensive case management, service coordination, and crisis response for adults with substance use or co-occurring disorders. Essential Responsibilities Provide substance use assessment support, psychoeducation, recovery planning, relapse-prevention, and disease-management interventions. Assist the Team Lead with behavioral health and substance use treatment interventions consistent with credential and scope of practice. Participate in development, implementation, and revision of person-centered plans. Coordinate and provide intensive case management, linkage, referral, monitoring, and follow-up. Help consumers access appropriate mental health, substance use, medical, housing, vocational, legal, and natural supports. Complete functional needs assessments and use findings to guide individualized interventions. Support housing stability, independent living skills, community integration, and recovery goals. Assist with crisis intervention and participate in the team's assigned first-responder coverage rotation. Communicate consumer progress and intervention effectiveness to the Team Lead and multidisciplinary team. Complete accurate, timely, Medicaid-compliant service documentation. Required Qualifications Current North Carolina credential as a CADC, LCAS, LCAS-A, or CCS, consistent with NC Medicaid CST requirements. At least one year of experience serving adults with mental health, substance use, or co-occurring disorders. Knowledge of recovery-oriented care, relapse prevention, crisis intervention, person-centered planning, and community resources. Ability to work independently in community settings and collaboratively within a multidisciplinary team. Preferred Qualifications ASAM training and experience applying ASAM Criteria. Experience with CST, ACT, peer support, intensive case management, or community-based substance use services. Familiarity with NC Medicaid and Trillium documentation and service requirements. Schedule and Availability This is a dedicated CST position, meaning the employee will be assigned exclusively to this one team. It does not guarantee 40 hours of work. The position averages 30 hours or less per week, and actual hours vary based on consumer needs, caseload development, and assigned team coverage. Work may include daytime, evening, weekend, and assigned on-call or crisis-response coverage. General Employment Requirements Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area. Ability to complete required background, health, credentialing, and payer enrollment checks. Ability to maintain timely, accurate, and Medicaid-compliant documentation in the electronic health record. Ability to work collaboratively with adults who have complex mental health, substance use, housing, medical, safety, vocational, and legal needs. Completion of all CST model, crisis-response, evidence-based practice, and agency-required training within established timeframes. About the Service Community Support Team is an intensive, community-based service for adults with mental health, substance use, or co-occurring disorders and complex treatment needs. The team provides treatment, rehabilitation, intensive case management, service coordination, skill building, crisis intervention, and support for successful community living.
