Job Description
Under general supervision of the Behavioral Health Services Program Manager, connects individuals to community agencies and resources; assist with navigation of complex systems to access behavioral health, mental health or other social services and supports. The Peer Navigator I must possess excellent oral and written communication skills.
- Must be at least 18 years old; High School Diploma or GED required.
- At least one year of education or experience in the field of substance abuse or mental health programs, or related field.
- At least one year of case management experience or related field preferred.
- Must successfully complete training for CPSW certification and CCSS within the first two years of employment.
- At least one year of demonstrated active recovery from substance abuse and/or mental illness preferred.
- Valid driver's license and ability to be covered by County Vehicle Insurance policy.
- Work is performed in an office and out in the community as required.
- May be subject to repetitive motion such as typing, data entry, and vision to monitor.
- May be subject to extended periods of intense concentration in the review of documents and reports.
- Work schedule for this position may include working on religious holidays.
- Utilize, process, and navigate the County's electronic ERP system as necessary and appropriate based on the needs and requirements of this position.
- Conduct client intake process and interview clients to determine priority of needs; facilitate connections to agencies and resources based on client's choices.
- Educate client and family member in understanding agencies, resources, and eligibility requirements.
- Participate in collaboration with community agencies and resources to support client's successful application and subsequent enrollment. Participate in inter-disciplinary teams to support clients with complex resource needs or logistical barriers. Communicate and collaborate with agencies to follow up on client connection status.
- Develop proficient knowledge of community agencies and resources, keep current record of agency profile and continuously update Resource Directory.
- Participate with staff to identify solutions and options to overcome behavioral health disparities and close the gaps that exist within the County.
- Promote Mental Health and Behavioral Health wellness by connecting clients seeking support in treatment or recovery to appropriate resources in the community.
- May participate in off-site events, health fairs, and outreach initiatives as assigned.
- Screen and enroll eligible clients in Medicate and other financial assistance programs.
- Develop and maintain accurate case files on all issues relating to client, complete required forms, and maintain and update database relative to connections to resources.
- Produce written and data-driven reports.
- May provide occasional transportation for clients when no other transportation options are available, coordinate and obtain approval from director.
- Handle sensitive and confidential data and ensure the quality and integrity of all produced information.
- Perform other duties as assigned.
