Salary Range:

$65,000-$71,000 We are seeking a Behavioral Health Case Manager to join the primary care clinic of a community health center. In this role, you will deliver high-quality behavioral health interventions and services that help patients access care and stay engaged in it. You will work closely with a multidisciplinary care team to coordinate services, support patients through crises, and connect them with the resources they need for positive health outcomes.

Key Responsibilities :

Provide proactive behavioral health case management, including comprehensive assessments, reassessments, and individualized care plans that ensure access to care and other needed services. Communicate regularly with the multidisciplinary care team to support positive patient health outcomes. Provide crisis intervention, stabilization, and brief counseling to patients as needed. Coordinate and link patients with additional services. Represent the health center in the community to promote services and engage new patients, particularly low-income individuals and communities of color, including participating in community events and tabling as needed. Serve as a liaison between patients and the multidisciplinary team to support care coordination and promote referrals to internal and external behavioral health providers. Respond promptly to phone, email, and referral inquiries about available services and other requests for information. Attend internal and external meetings and trainings. Keep patient records current in real time, organized, and legible, and complete and submit required forms, reports, and summaries on time. Maintain accurate records of services provided. Ensure electronic and paper records meet all documentation and data reporting requirements.

Requirements:

Master's degree in mental health counseling or social work. At least two years of experience in a related field. Verifiable internships and unpaid volunteer work count toward this requirement.
