Position Overview:

We are seeking a motivated and empathetic CDCA Case Manager to join our IOP and PHP team. The ideal candidate will play a crucial role in supporting clients through their recovery journey by providing case management services, co-facilitating group sessions, and ensuring clients receive the resources and care they need.

Key Responsibilities:

Provide case management services to clients participating in IOP and PHP programs. Co-Facilitate group therapy sessions focused on substance use education, coping skills, and recovery support. Collaborate with a multidisciplinary team to create and implement individualized treatment plans. Assist clients in accessing community resources such as housing, employment, and medical care. Monitor client progress and document services in compliance with agency and regulatory standards. Advocate for clients to ensure they receive equitable care and support. Maintain knowledge of and compliance with CDCA ethical guidelines and best practices. Support clients in navigating challenges in early recovery and developing relapse prevention strategies. Participate in team meetings, training, and supervision as required.

Qualifications:

Valid CDCA certification in the state of Ohio. Experience working with clients in behavioral health or substance use treatment settings preferred. Strong organizational and time-management skills. Ability to build rapport with clients and work collaboratively with a diverse team. Knowledge of community resources and support systems.
