Job Description
Who we are Stepping Stone is a behavioral health agency specializing in long-term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assist the residents transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, behavior management, substance use and other behavioral health issues. Our team delivers personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health and substance use needs are met with compassion and expertise. ( Compensation will vary based on experience and if applicants also hold a valid CDCA license in the state of Ohio. ) Responsibilities may include: Develop and implement individualized case management plans that align with clinical guidelines and patient goals. Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs. Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan. Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services. Provide services to individual clients or groups of clients. Promote a positive and cooperative relationship with all outside contacts. Collaborate with social workers, addiction counselors, medical staff, psych staff and discharge planners to arrange care services and outpatient support. Maintain accurate electronic records while ensuring compliance with HIPAA regulations and confidentiality standards. Provide patient education on mental health conditions, medication adherence, and community resources to promote recovery and wellness. Skills Proven expertise in behavioral health, mental health counseling, addiction counseling, and social work within inpatient or outpatient settings. Strong knowledge of medical terminology, physiology, patient assessment techniques, and diagnostic evaluation processes. Experience working with diverse populations in nursing homes and assisted living facilities. Skilled in discharge planning, case management strategies, and medical records documentation. Ability to navigate HIPAA regulations diligently while managing sensitive patient information.
