Job Summary We are seeking a dedicated and compassionate SUD (Substance Use Disorder) Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive care plans for individuals struggling with substance use disorders, ensuring they receive the appropriate treatment, support, and resources to foster recovery and improve their quality of life. Duties Conduct behavioral health screenings, to determine individual needs and treatment goals. Develop and implement personalized case management plans that incorporate addiction counseling, mental health support, discharge planning, and community resource coordination. Collaborate closely with healthcare providers across inpatient, emergency medicine, crisis care providers and trauma centers to ensure seamless care transitions and continuity of treatment. Manage electronic health records (EHR) efficiently while maintaining compliance with HIPAA regulations to protect patient confidentiality. Facilitate intake processes for new clients, ensuring comprehensive documentation of medical history, substance use patterns, and social determinants affecting health outcomes, . Provide patient service by offering education on physiology related to addiction and mental health conditions while supporting families through counseling and discharge planning efforts. Non-12 Step group facilitation, inhouse training, CBT, DBT, and mindfulness. Skills Strong knowledge of clinical case management practices in behavioral health and substance use disorders. Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds and those with mental health conditions or disabilities. Familiarity with electronic health records (EHR/EHR management systems), HIPAA compliance standards, and utilization management processes. Ability to prioritize tasks efficiently while managing multiple caseloads in an open door, fast-paced environment. Relevant certifications including, CPR, addiction counseling credentials.
