Job Description

DUTIES AND RESPONSIBILITIES
include but not limited to the following: Respond to individuals with behavioral health crises to stabilize the individual in their community, and transition them into appropriate wrap-around services that supports long-term recovery and stability; services typically range from 3 to 7 days Support CS clinical staff with comprehensive mental health assessments including mental status exams, suicide/homicide risk evaluations, and substance use screenings Utilize evidence-based de-escalation techniques to stabilize individuals in crisis Implement and review individualized safety plans and facilitate crisis resolution planning Coordinate and consult with multidisciplinary teams, including psychiatrists, nurses, case managers, law enforcement and EMS personnel, etc Complete intake documentation, progress notes and discharge summaries in collaboration with MCR team partners Maintain up-to-date client records and prepare required reports in a timely and accurate manner EDUCATION and/or
EXPERIENCE
To qualify a candidate MUST have at least a bachelor's degree in human service field or special education from an accredited college and with at least one year of experience and a
VALID QMHP
Certification.
TRAINING
Mobile Crisis Training Certificate is Preferred.
Staff must be certified in:
CPR, First Aid, TOVA and become orientated to the company's guidelines, policy and procedures, and take part in staff trainings as they occur.
Additional Requirements:
Employee must be certified as tuberculosis free Employee must clear criminal background and CPS registry checks
TRANSPORATION
Case Managers are required to use their personal vehicles for visits and for engaging in community activities. Therefore, at the time of hire Case Managers must provide proof of Driver's license and proof of Automobile Insurance.
Case Managers are required to:
Provide safe, timely, and appropriate transportation for clients as required, including discharge transport from hospitals, emergency departments, or crisis receiving facilities to designated residences, programs, or approved placements. Ensure all transportation activities are conducted in compliance with agency policies, safety protocols, and confidentiality standards. Assess client readiness for transport and coordinate with hospital staff and treatment teams prior to discharge. Maintain professional boundaries and ensure client safety during transport, including de-escalation support if needed. Document all transportation activities accurately and in accordance with program and regulatory requirements. Maintain a valid driver's license, acceptable driving record, and active automobile insurance.

$35.00 per hour
