80-90% regional travel required Position Summary We are seeking an experienced Child & Youth Behavioral Health Intensive Case Manager to provide comprehensive healthcare management services to children and families throughout the Jefferson County region of Kentucky. This position is responsible for supporting appropriate and effective healthcare treatment, facilitating optimal member outcomes, coordinating services, managing healthcare resources, and helping improve overall program and operational efficiency. The Case Manager will use critical thinking, professional judgment, advocacy, collaboration, coordination, education, and motivational interviewing to identify member needs and connect individuals and families with appropriate healthcare and community resources. This is a field-based position requiring approximately 80-90% travel throughout the assigned region. The successful candidate will meet members face-to-face in settings such as homes, schools, hospitals, doctors' offices, and social service agencies. Candidates must reside in Jefferson County, Kentucky and have reliable transportation. Travel to the Louisville area for meetings and training may also be required. Key ResponsibilitiesHealthcare Management & Member Assessment Provide comprehensive healthcare management services to members with complex healthcare and behavioral health needs. Evaluate members to determine appropriate healthcare services and support. Assess medical necessity and appropriateness of services based on established guidelines, member needs, benefits, and available resources. Identify high-risk factors, barriers to care, and service needs that may impact member outcomes. Develop, coordinate, implement, and evaluate individualized care plans. Monitor member progress and adjust interventions as appropriate. Facilitate optimal healthcare outcomes while supporting quality, safety, and member satisfaction. Identify opportunities to improve healthcare program effectiveness and operational efficiency. Help optimize healthcare resources and total costs while ensuring appropriate care is provided. Member Advocacy & Education Advocate for members and help them actively participate in healthcare decision-making. Educate and empower members and families to understand their healthcare options, services, benefits, and responsibilities. Use motivational interviewing and engagement techniques to encourage positive lifestyle and behavioral changes. Provide coaching, education, information, and support to promote member independence and self-management. Help members work effectively with their healthcare providers and other service professionals. Promote compliance, satisfaction, and patient/member advocacy. Collaboration & Quality Collaborate with case managers, supervisors, medical professionals, and community partners to address barriers to care. Participate in multidisciplinary case conferences and reviews when appropriate. Identify and escalate quality-of-care concerns through established processes. Identify opportunities for continuous quality improvement and regulatory compliance. Recognize potential service or process variances and participate in appropriate corrective action when needed. Apply organizational policies, program directives, and established guidelines to support consistent and appropriate decision-making. Support colleagues and team members by sharing knowledge and helping promote appropriate decision-making at the appropriate level. Documentation & Compliance Maintain accurate and timely documentation of assessments, member interactions, care plans, interventions, and outcomes. Follow case management and quality management processes in accordance with applicable regulatory and accreditation requirements. Ensure activities and decisions are consistent with established policies, procedures, and program requirements. Monitor, evaluate, and document the effectiveness of care management interventions. Required Qualifications Bachelor's or Master's degree in behavioral health or a relevant human services field, such as: Social Work Psychology Marriage and Family Therapy Counseling Other related human services fields Minimum 5 years of relevant professional experience preferred for this role. Minimum 2 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling. Minimum 6 months of experience in one or more of the following: Foster care or juvenile justice systems Adoption assistance Behavioral health services Trauma-informed care Adverse Childhood Experiences (ACEs) Crisis intervention services Evidence-based practices applicable to Kentucky SKY populations Minimum 1 year of experience with Microsoft Office Suite , including Word and Excel. Experience providing case management, healthcare management, behavioral health, or social service support. Strong communication, organization, critical-thinking, and problem-solving skills. Willingness and ability to travel 80-90% of the time throughout the assigned region. Reliable transportation required. Must reside in Jefferson County, Kentucky. Ability to work occasional hours outside of standard business hours based on member and family needs. Preferred Qualifications 3+ years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling. 1+ year of Managed Care Organization (MCO) experience. Experience working with children, families, and complex behavioral health needs. Experience with healthcare management, utilization management, quality improvement, or care coordination. Experience working with Kentucky SKY populations. Schedule Monday-Friday, 8:00 AM-5:00 PM EST Flexibility to work until 7:00 PM may be required based on member and family needs. This may include appointments or services scheduled after school or after traditional business hours. Compensation $30.00 per hour Position Details This is a field-based position serving members throughout the Jefferson County region. The majority of the workday will be spent traveling within the assigned region and meeting members face-to-face.
