We are seeking a compassionate, dependable, and motivated Independent Contractor - Qualified Mental Health Specialist (QMHS) / Case Manager to join our growing team. The ideal candidate is passionate about helping individuals achieve their personal, behavioral health, and life goals and is comfortable providing services in community-based and/or virtual settings.
Position Summary:
The QMHS/Case Manager will provide individualized support, care coordination, case management, skill development, advocacy, and community-based services to individuals receiving services through Empowered Mentoring. The contractor will work collaboratively with individuals served, families/support systems when appropriate, community providers, and other members of the individual's service team to promote stability, independence, personal growth, and achievement of identified goals.
Responsibilities:
Provide individualized mentoring, case management, and supportive services. Assist individuals with identifying and achieving personal and treatment-plan goals. Provide skill-building and community-based support. Connect individuals with appropriate community, healthcare, housing, employment, and other resources. Coordinate services with providers and support systems. Advocate for individuals and promote independence. Complete accurate and timely documentation in the EHR. Maintain confidentiality and professional boundaries. Communicate concerns, changes in needs, and safety issues appropriately. Participate in required meetings, training, and supervision.
Qualifications:
Must meet applicable requirements to provide services as a Qualified Mental Health Specialist (QMHS). High school diploma or equivalent required. Experience in behavioral health, case management, mentoring, social services, or a related field preferred. Strong communication, organization, and documentation skills. Ability to work independently and professionally. Reliable transportation may be required for community-based services. Must be able to pass required background checks and maintain required credentials/training.

Communicate consistently with Empowered Mentoring Group regarding service needs and concerns. Maintain reliable availability and professional communication. Follow applicable laws, regulations, contractual requirements, and organizational policies.
