A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

The Responsibilities Holistic Assessment:
Conduct comprehensive evaluations of medical, behavioral, and social needs, utilizing a Human Services lens to see the whole person.
Tailored Care Planning:
Design individualized care plans that respect each patient's unique history while ensuring alignment with clinical goals and patient preferences.
Resource Linkage & Advocacy:
Drive household stability by securing essential community resources, including housing retention, utility assistance, SNAP, and WIC.
Economic Empowerment:
Guide clients toward financial independence through steady support in employment searches and vocational development.
System Navigation:
Act as the primary link between patients, community supports, and outpatient services, ensuring continuity of care. Integrity in
Documentation:
Maintain impeccable medical records and case notes in compliance with Medicaid billing standards , HIPAA regulations, and professional excellence.
The Skill Set Educational Foundation:
A Bachelor's degree in Human Services , Social Work, Psychology, or a related behavioral science field is required.
Licensing & Certification:
Must possess a valid TCM certification
Community Expertise:
Proven success in Human Services delivery , including home-based care, community outreach, or telehealth settings.
Strategic Knowledge:
A fluent command of social determinants of health, patient assessment techniques, and community resource systems.
Refined Communication:
The ability to engage with families, providers, and stakeholders with both warmth and professional poise.
Clinical Insight:
Experience with behavioral health assessments or social work that allows for proactive, patient-centered care.
